Doctors at Evergreen Practice have decided to offer Summary Care Records (SCRs) to patients.
SCRs contain a limited amount of information about a patient’s current and repeat medications, allergies and poor reactions to certain medicines.
They are designed solely for use when you are treated away in an emergency or if you need care when the practice is closed – either at a hospital, walk-in centre or by an out-of-hours GP service.
The emergency care summaries are only viewed when a patient requires treatment and the patient must agree to their SCR’s use unless they’re unconscious or deemed incapable of making a decision in the best interests of their healthcare.
